The death of a loved one may require you to notify various organizations. To simplify the process we have created sample letters that can be personalized to notify organizations about the death, and to initiate the claims process. Be sure to:
- Include your full name, address and phone number
- Keep a copy of every letter
- Include deceased’s full name, full date of birth and death
