Announcing the return of our It’s EZ Being Green® Sweepstakes

AXA Equitable proudly continues our commitment to providing you with your financial documents in an easy, secure, timely and eco-smart manner. With a growing awareness of the resources consumed in printing and mailing these documents, you also have the option to receive certain information digitally with our Electronic Delivery System (eDelivery).

The Sweepstakes

Beginning January 1, 2012 and running until year-end, we’re bringing back our It’s EZ Being Green^® Sweepstakes and making it bigger and better than ever, with:

• Cash prizes
• More winners throughout the year

By enrolling your AXA Equitable variable annuity or variable life policy into our eDelivery system or sending us a postcard entry,* you’ll have the opportunity to enter the sweepstakes. If you have a policy, it only takes a few minutes to sign up and you’ll become eligible for a chance to win one of the cash prizes below:

• 4 Grand Prizes (one to be awarded per Entry Period)
  $10,000.00 cash prize 
• 16 First Prizes (four to be awarded per Entry Period)
  $500.00 cash prize
• 100 Second Prizes (25 to be awarded per Entry Period)
  $20.00 gift card

How to Enter

If you are not yet enrolled in eDelivery:

• Log into your account through online account access at axa-equitable.com. If you are not currently registered, click here to register now. It only takes a moment to create a new user name and password.
• After logging in, click eStatements from any policy drop-down.
• Click "Delivery Preferences."
• Select at least one document to enroll in eDelivery.
• Go back to the Account Summary page and click on the "Enroll in Sweepstakes" button to the right.

If you are already enrolled in eDelivery:

Simply log into your account and click the Sweepstakes promo banner to your right to confirm your entry.

A PURCHASE DOES NOT IMPROVE YOUR CHANCES OF WINNING. *Sweepstakes runs from Entry Period First 12:01 a.m. ET on 1/1/12 through 11:59:59 p.m. ET on 3/31/12; Second 12:01 a.m. ET on 4/1/12 through 11:59:59 p.m. ET on 6/30/12; Third 12:01 a.m. ET on 7/1/12 through 11:59:59 p.m. ET on 9/30/12; Fourth 12:01 a.m. ET on 10/1/12 through 11:59:59 p.m. ET on 12/31/12. To enter this Sweepstakes without enrolling a Policy for eDelivery Service, prepare a plain 3 inch x 5 inch index card on which your first name, last name, date of birth, complete address, city, state, zip code, home phone number (including area code) and e-mail address is legibly hand-printed or typed, and mail your completed 3 inch x 5 inch card to: "It’s EZ Being Green^®" – 90 Rockwood Place, Rochester, NY 14610. To be considered eligible for the First Entry Period, Alternate Method of Entry (AMOE) entries must be postmarked no later than March 31, 2012 and received no later than 5:00 p.m. ET on April 7, 2012. To be considered eligible for the Second Entry Period, AMOE entries must be postmarked no later than June 30, 2012 and received no later than 5:00 p.m. ET on July 7, 2012. To be considered eligible for the Third Entry Period, AMOE entries must be postmarked no later than October 1, 2012 and received no later than 5:00 p.m. ET on October 8, 2012. To be considered eligible for the Fourth Entry Period, AMOE entries must be postmarked no later than December 31, 2012 and received no later than 5:00 p.m. ET on January 7, 2013. Limit one (1) entry per person per Entry Period.
