Claim for Death Benefits

Please accept our condolences on your loss. Because this is a difficult time, we want to assist the beneficiary of life insurance benefits by quickly processing a request. According to the policy's provisions, the benefit amount payable will be determined as of the date of the insured's death. A lump sum payment of insurance benefits is made through our Access Account, a money market type account with checkwriting privileges. Details about this account are included in the instructions provided below.
